Taylored Health One-on-One Coaching

Welcome! During the course of Nutrition Coaching, Client will be guided through various topics that will help empower him/her to embrace health practices from an informed, supported place with clarity and confidence.

This Agreement is made between the Coach and Administrator of Taylored Health, Taylor Wessel, and the person named at the end of this document, The Client. One-On-One Coaching that you are enrolling in includes the following:

• Zoom meeting(s)

• Follow-up email with action plan, video recording and applicable resources (i.e. product recommendations, recipes and videos)

• Email support as agreed upon between Coach and Client

PAYMENTS AND REFUNDS:

The Client understands and agrees to pay the cost of One-On-One Nutrition Coaching Session(s) in full.

Should the Client need to cancel this session, for whatever reason, the Client will remain responsible for the balance of the Nutrition Sessions unless cancellation notice is received 48 hours prior to the intended session. By signing this Agreement, the Client agrees to be legally obligated to pay the full amount for Nutrition Coaching.

One-On-One Nutrition Coaching Sessions expire 12 months after activation.

COMMITMENT:

This Agreement is for a relationship of stated duration between Coach and Client, designed to facilitate and improve the Client’s personal health practices in a group format with other Clients.

Coaching is for individuals who are emotionally and psychologically healthy. Coaching is not advice, therapy or counseling.

The Client understands that the role of the Health Coach is not to prescribe or assess micro-and macronutrient levels; provide health care, medical or nutrition therapy services; or to diagnose, treat or cure any disease, condition or other physical or mental ailment of the human body. The Coach is a mentor and guide who has been trained in health coaching to help clients reach their own health goals by helping them with up-to-date information, facilitating new practices and instituting sustainable lifestyle changes.

Should Client be under the care of a medical professional, Client agrees to consult any dietary changes with their doctor and should not discontinue use of any prescription medication without first consulting their physician.

Client acknowledges that the best way to see results is to commit to the Coaching practice by fully engaging in all preparatory and homework materials as guided by the Coach.

COACHING SESSION PROCEDURES:

All coaching sessions are conducted by telephone/internet unless otherwise arranged with the Coach. If the Client is unable to attend the scheduled session time, the Coach requires 48 hours of notification in order to reschedule without penalty.

CONFIDENTIALITY:

Conversations within coaching sessions are confidential and will be protected as such. Information will be shared outside of sessions only with written consent from Client or if required by law. Additionally, Client understands that Coach will not abide by the Confidentiality provision in circumstances where the Coach: (i) learns of child abuse or neglect or, (ii) imminent harm to self or an individual(s).

DISCLAIMER:

Coaching is meant to challenge, uplift, and support Client psychologically. However, coaching is not psychotherapy nor is it being offered as a substitute for mental health care. If Client feels psychologically stressed to the point that it is interfering with her ability to function, Client agrees to seek the help in the form of a professional counselor trained for that specialty.

Client waives and releases Coach from all actions, claims, or demands that Client, its assignees, heirs, guardians, and legal representatives now have or may hereafter have for injury, illness, death or damage to property resulting from or relating to her participation in coaching sessions contemplated by this Agreement.
