Consent to Work with a Tutor
I certify that I am the parent or legal guardian of the child whom I am registering for tutoring through Illini Tutoring, or that I am the student and I am 18 years or older, or I am the person responsible for payment. I am giving permission for my child, or I am giving my own consent, herein referred to as "the student," to work one-on-one with a tutor at 2500 Galen Drive, or at another public location or online using Zoom or Google Hangouts should circumstances arise that preclude holding a tutoring session at 2500 Galen Drive. I understand that it is my responsibility to have the student at the Illini Tutoring office ready to start his/her tutoring session on time. I understand that Illini Tutoring can make no guarantees with regards to test scores and grade improvement, and/or essay writing or progress or acceptance to colleges,scholarships, universities, high schools, any other institutions as a result for any math, science, academic coaching, academic tutoring, SAT, ACT, PSAT, SSAT tutoring, any other test prep tutoring, college application essay writing or any other writing tutoring, and I will not hold the company, the owners, the employees, or any agents of Illini Tutoring responsible in the event that my child does not benefit from the services provided. I understand that Illini Tutoring does not offer full or partial refunds for unused tutoring except when Illini Tutoring deems it necessary to discontinue tutoring with a student, and Illini Tutoring LLC retains the right to grant refunds at its full discretion.
I understand that tutoring hours are not transferable or refundable after payment has been made at registration.
Rescheduling Policy
I understand that it is my responsibility to reschedule my tutoring sessions, or my child's tutoring sessions, from the scheduling system, Acuity(the emailed product receipt can be used to schedule all sessions). All reschedules must be made at least 24 hours in advance for any reason. Failure to reschedule tutoring sessions at least 24 hours in advance means the tutoring hour will be forfeited. It will not be made up, and it will not be refunded regardless of the reason provided.
Tutoring Packages and Payment
Parents/guardians/students agree to pay in-full at registration for their package of tutoring. They may increase their tutoring plan, but they may not decrease their tutoring plan. All spring semester math and science tutoring packages of 16 hours or more must be used between January 7, 2024 and May 27, 2024. All summer math and science program sessions must be used between June 3rd, 2024 and August 16th, 2024. Packages of 6 tutoring hours for all tutoring must be completed within 60 days of purchase. Sessions may not be rolled over, or carried over, to a later date or future semester. There are no refunds for unused tutoring sessions. If sessions can't be scheduled or rescheduled by a given date due to instructors' full availabilities, sessions will not be carried over to a future term, nor credited, and refunds or partial refunds would be provided.
Packages for all tutoring sessions must be completed within the days of indicated on these terms and/or the registration page on the website at the time of purchase, including for all other tutoring sessions(math and science, test prep, college essay writing, academic coaching, life coaching, etc). Any other expiration date(s) that may appear after purchase(including on the scheduling page or the product receipt of payment) will be disregarded.
On-Time Payment Policy(continued)
Any other payment arrangement must be agreed by Illini Tutoring in writing. Any exception to any term in this contract must not be construed to mean the contract is null and void. All terms of the contract will be enforced through the end of the student's tutoring package, as stated in this contract.
Illini Tutoring LLC is committed to providing a work- and learning environment free of sexual or any other form of unlawful harassment or discrimination. Illini Tutoring LLC has a no-tolerance policy, and is committed to maintaining a respectful, courteous work and learning environment in all work- related settings that respects the dignity and worth of each employee and student. Inappropriate behavior and unlawful harassment are wholly inconsistent with this commitment. No employee, client, parent, student, vendor or other person who does business with Illini Tutoring LLC is exempt from the prohibitions within this policy.Harassment or discrimination based on race, color, ethnicity, sex (including pregnancy, childbirth, breastfeeding, or related medical conditions), gender (including gender identity and gender expression), religion (including religious dress and grooming practices), religious creed, marital status, age, national origin, ancestry, citizenship, physical or mental disability, military and veteran status, genetic information, medical condition (including cancer and genetic characteristics), sexual orientation, or any other protected class, characteristic, or consideration made unlawful under applicable federal, state, or local laws, is illegal and prohibited by Illini Tutoring LLC policy. Such conduct by or toward any employee, client, parent, student, vendor or anyone else who does business with the Company will not be tolerated.
We believe that the strength of Illini Tutoring LLC is found in the diversity of our staff and our students. We actively employ and serve people from diverse backgrounds. Our employees and subcontractors receive ongoing training on cultural competency and creating an inclusive atmosphere that is respectful of all people.
When a client (student or parent) is found to be harassing, bullying, or intimidating another student, parent, or employee of Illini Tutoring LLC, we reserve the right to immediately terminate this contract and discontinue providing services to the student and/or the parent. We may refund a portion of the student's tutoring payments for unused tutoring that would be canceled. If a refund is given, the maximum refund given will exclude the non-refundable $250 deposit payment.
Sexual or other unlawful harassment or discrimination includes any verbal, physical, or visual conduct based on sex, race, age, national origin, disability or any other legally protected basis if:
Sexual harassment includes unwelcome sexual advances, requests for sexual favors, and lewd, vulgar, or obscene remarks, jokes, posters, or cartoons, and any unwelcome touching, pinching, or other physical contact. Other forms of unlawful harassment or discrimination may include racial epithets, slurs, and derogatory remarks, stereotypes, jokes, posters, or cartoons based on race, national origin, age, disability, marital status, or other legally protected categories. Prohibited harassment also includes the transmission of materials such as cartoons, jokes, or links to offensive materials online using the Illini Tutoring LLC’s electronic communications system, or through other online conduct.Treating any employee or individual differently or unfairly because of the characteristics listed above is also prohibited. Unlawful discrimination includes conduct that is based on the perception that anyone has any of those characteristics, or is associated with a person who has or is perceived as having any of those characteristics. All Illini Tutoring LLC employees, other workers, and representatives are prohibited from engaging in unlawful discrimination.
Test Prep Agreement
The adult student, or the parent or legal guardian registering for the minor student, is responsible for scheduling all tutoring hours. All reschedules are the responsibility of the adult student or their parent or legal guardian responsible for the student. Tutoring hours must be rescheduled at least 24 hours before the start of the originally scheduled tutoring hour. Tutoring hours that aren't rescheduled at least 24 hours in advance are forfeited and will not be made up.
Tutor has the right to set additional limitations for individual students on rescheduling if excessive rescheduling impacts the tutor's ability to deliver effective tutoring services.
By agreeing to these terms, you acknowledge and agree that Illini Tutoring may grant an exception one time to the 24 hour rescheduling notice at the discretion of the tutor and/or Illini Tutoring LLC. When an exception is granted to the rescheduling policy, it does not change the terms or conditions of rescheduling for future sessions.
