Scheduling Time - To ensure that your time, as well as the time of my other students, is respected, appointments will begin promptly at their pre-scheduled time regardless of your arrival time.
Deposit and Cancellation Notice - Any student who cancels an appointment with less than 24-hour notice will be charged for the lesson.
Fees - Payment is due at the beginning of the session.
Obligation of the Student - The student shall assist the tutor in identifying problem areas that need specific tutoring. The student agrees to provide his or her own materials for each tutoring session (e.g., books, notes, calculator, etc.). The student is expected to prepare for the session by determining what needs to be covered, sharing that information with the tutor, and participating actively in all aspects of the tutoring session by asking questions, completing homework, etc.
No warranties - While confident in her/his skills and teaching ability, the tutor makes no promises or warranties regarding a student's performance as a result of the tutoring provided.
Termination - Either party may terminate this tutoring contract at any time by giving the other party seven days' prior notice.
Confidentiality - I will not release any information to anyone without written authorization from you, except as required by law. You have the right to access my written record about you or a summary of it.
Redress - My name is Sabine Charles, Certified Hypnotist C.Ht. I am a certified member of the National Guild of Hypnotists and practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P.O. Box 308, Merrimack, NH 03054-0308, phone (603) 429-9438, to seek redress. Other services than my own may be available to you in the community. You can locate such providers in the telephone book. (Applicable only to hypnosis clients).
