Allure Lash & Beauty Co. Membership Agreement

1. Services and Membership Terms

1.1 The Client agrees to a membership with Allure Lash & Beauty for the services described in this contract.

2. Invoices and Payments

2.1 Allure Lash & Beauty will invoice the Client every 30 days from the original sign-up date unless otherwise noted in the Payment schedule.

2.2 The Client agrees to pay invoice(s) by the due date(s) specified. Unpaid or overdue invoices may result in suspension or termination of the membership.

2.3 Payment will be made to Allure Lash & Beauty via cash, check, an approved payment card, or any other payment method determined by Allure Lash & Beauty.

3. Refunds

3.1 No refunds will be provided. The Client will be assumed to have accepted the service unconditionally, unless a claim that the service is defective is made within 24 hours from the date of service.

4. Licensing

4.1 Allure Lash & Beauty holds all licenses necessary to perform the work. All work will be done in compliance with all applicable federal, state, or local laws and regulations.

5. Authority to Sign

5.1 Each party has the authority to enter into this Contract and to perform all of its obligations under this Contract.

6. Termination of Contract

6.1 This agreement will continue month to month and cancellation will be permitted with 30 days written notice.

6.2 The Written Notice of Cancellation will be on a form supplied by Allure Lash & Beauty Co. and may be submitted via mail, email, or hand delivery.

6.3 After the Notice of Cancellation is submitted, one final monthly payment will be charged on the next regularly scheduled payment date.

7. Membership Duration

7.1 Allure Lash and Beauty Co. offers membership programs that require a minimum commitment period of six (6) months from the date of enrollment.

7.2 After the initial six-month commitment period, memberships will automatically continue on a month-to-month basis, unless a cancellation notice is provided by the member.

8. Late Fee

8.1 In the event that the payment for the membership is not received within 5 days of the due date, the Member shall be subject to a late fee of $25.

8.2 If the payment extends beyond 6 days past the due date, an additional late fee of $25 will be applied, resulting in a total late fee of $50.

9. Modifications

9.1 Allure Lash & Beauty may update and make changes to this contract at any time, with a written notice or email within 30 days prior to the change.
