Scheduling Time - To ensure that you time is respected, as well as the time of my other students, appointments will promptly begin at their pre- scheduled time regardless of the time of your arrival.
Deposit and Cancellation Notice - Any student that cancels his or her appointment with less than a 24-hour notice will be charged for the lesson
Fees - Payment is due at the beginning of the session.
Obligation of the Student - The student shall assist the tutor in identifying problem areas in which the student needs specific tutoring. The student agrees to provide his or her own materials needed for each tutoring session (e.g. books, notes, calculator, etc.). The student will prepare for the session by determining what needs to be covered and sharing that information with the tutor by participating actively in all aspects of the tutoring session by asking questions, completing homework, etc.
No warranties - While the tutor is confident in her/his skills and teaching ability, the tutor makes no promises or warranties about a student’s performance as a result of any tutoring provided.
Terminations - Either party may terminate this tutoring contract at any time by giving the other party seven days prior notice.
Confidentiality – I will not release any information to anyone without a written authorization from you, except as provided for by law. You have a right to be allowed access to my written record or a summary of my record about you.
Redress – My name is Sabine Charles, Certified Hypnotist C.Ht. I am a certified member of the National Guild of Hypnotists, and practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P. O. Box 308, Merrimack, NH 03054-0308, (603) 429-9438, to seek redress. Other services than my own may be available to you in the community. You may locate such providers in the telephone book. (Only Applicable for Hypnosis Clients)
