By accepting these terms & conditions you are agreeing to the Relax Bodyworks cancellation policy requiring a minimum of 12 hours notice of appointment cancellation, any less and 50% of the service total will be charged to the credit card use to secure the appointment. No showing a Gift Certificate appointment will result in the voiding said Gift Certificate, as well as any charges to cover any pricing differences or additional service enhancements. Also by agreeing to these terms, you are acknowledging that your massage therapist can not diagnose any illness, nor prescribe any treatment. You also acknowledge that full sheet draping will be utilized during all sessions and that should you become uncomfortable at any time, you may end the session for any reason. All clients under the age of 18 must be accompanied by a guardian for their first appointment in order to obtain written consent for service. Lastly, by accepting these terms you acknowledge that this service is NON SEXUAL in nature, any proposition for sexual services will result in terminating your massage session Immediately, you will be charged fully for the appointment, and you will be banned from scheduling with Relax Bodyworks in the future. It will also be at the discretion of your therapist as to whether or not your name and contact information will be reported to local authorities.
