I understand and agree to the 24 hour Cancellation Policy for Private and Semi Private Lessons and if registering for a group program I agree to that particular program's Cancellation Policy.

While myself or the person I am registering is participating in golf clinics, lessons, schools and programs with St. Louis Golf Lessons LLC or Maria Palozola , I acknowledge and assume all the foregoing risks and likewise accept personal responsibility for any injury or damages that may occur. I release , waive, discharge and covenant not to sue St. Louis Golf Lessons LLC or Maria Palozola or other instructors/substitutes they may hire to conduct activities. .

I give my permission for myself or the person I am registering to be photographed, video taped, and/or interviewed for promotional purposes while attending St. Louis Golf Lessons LLC or Maria Palozola's golf lessons, clinics, schools and programs.

I acknowledge, to the best of my ability, that I or the person I am registering is in good health and have no known medical problems that would restrict participation in an exercise program and has had a recent physical examination with a doctor and was found able to undertake an exercise program.

I understand that there is no refund for private lessons, but that they may be transferred to the person of my choice.

I understand that all instructors are independent contractors and once paid by St. Louis Golf Lessons the instructor is solely responsible for completing all pre paid lessons with me.

I understand that all lessons expire 6 months from the date of purchase and all gift certificates expire 12 months from the date of purchase.
