Terms of Agreement for Online Scheduling

Welcome to ChiroFix Acupuncture & Functional Medicine’s online scheduling service. Please read the following terms of use carefully before using our online scheduling system. By accessing and using this service, you agree to be bound by these terms. If you do not agree to these terms, please do not use this service.

1. Eligibility You must be at least 18 years of age or have parental consent to schedule an appointment through our online scheduling system.

2. Scheduling

• You can schedule, reschedule, or cancel appointments with our healthcare providers through this platform.
• Appointments are subject to availability and are not guaranteed until confirmed by our staff.
• We recommend scheduling appointments at least 24 hours in advance to ensure availability.

3. Cancellations and No-Shows

• We require at least 24 hours' notice for appointment cancellations or rescheduling. Failure to cancel or reschedule at least 24 hours before your appointment may result in a cancellation fee.
• No-shows or repeated cancellations may result in restricted access to online scheduling or services.

4. User Responsibilities

• You are responsible for providing accurate and current information when scheduling appointments.
• You agree to arrive on time for your appointments. Late arrivals may result in shortened treatment time or rescheduling.

5. Privacy and Security

• We are committed to protecting your privacy. Personal information entered through the online scheduling system will be processed in accordance with our privacy policy.
• You are responsible for maintaining the confidentiality of your login information and for all activities that occur under your account.

6. Liability

• ChiroFix Acupuncture & Functional Medicine is not liable for any direct, indirect, special, incidental, or consequential damages arising out of the use or inability to use our online scheduling service.

7. Modifications to Terms We reserve the right to modify these terms at any time. Your continued use of the online scheduling system after any such modifications signifies your acceptance of the new terms.
