I understand that appointments are by phone.

I will not use a debit card.

I will follow instructions and right away book all 4 recurring weekly appointments to take place the same time and day.

I am not paying for appointments, but rather for a membership (whether I get 0, 1, 2, 3, or 4 appointments in a given membership period).

Memberships automatically renew unless canceled.

I must show up to my appointments.

I must cancel my appointment if I cannot make it.

If I need to reschedule an appointment, I need to do so at least 12 hours in advance.

The recurring membership fee may increase at some point in the future. In the event of a price increase, I will be notified well in advance.

Canceling a membership does not result in a refund of any previous payments. Canceling a membership only prevents future recurring payments from taking place.

It is my responsibility to cancel my membership through the scheduling system. Cancelation links may be found in any membership-related e-mail that I receive from Yossi Muller Nutrition.

Notifying Yossi or his office staff will not result in the cancelation of membership. I will still be required to cancel using the provided links in my emails.
