A £1.50 per mile travel charge will be added to distances over the first 10 miles (from Polesworth). If this is applicable to you, you will receive this via separate invoice prior to your consultation.

Notice for Behaviour Bookings: When you schedule your first consultation, you will receive a confirmation e-mail containing a PDF questionnaire and a Vet Referral Form. Please fill out the former and send back to me no less than 48 hours before your appointment. Please take the latter to your vet prior to meeting me and ask them to give your dog a full health check and sign the form - this enables me to liaise with your vet in future if needed.
