1. Acknowledgment of Risks:

I acknowledge that participating in assisted stretching services involves inherent risks, including but not limited to physical injury, strains, sprains, or other health complications. I understand that these risks can arise from my own actions, the actions of others, or the condition of the environment.

2. Health Declaration:

I certify that I am physically fit and capable of participating in assisted stretching services. I have consulted with a physician if necessary, and I do not have any medical conditions that would prevent me from participating in such activities.

3. Release of Liability:

In consideration of being allowed to participate in assisted stretching services, I hereby waive, release, and discharge Better Stretch LLC, its owners, employees, contractors, agents, and volunteers from any and all claims, demands, damages, and liabilities of any kind arising from my participation in these services, including any claims related to personal injury, property damage, or wrongful death, whether caused by negligence or otherwise.

4. Indemnification:

I agree to indemnify and hold harmless Better Stretch LLC from any claims made by third parties as a result of my participation in the assisted stretching services.

5. Governing Law:

This waiver and release shall be governed by and construed in accordance with the laws of the State of Illinois.

6. Severability:

If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

7. Acknowledgment of Understanding:

I have read this waiver and release of claims and fully understand its contents. I am signing this waiver voluntarily and intend to be bound by its terms.
