LB Utah Punch Pass

With an Open Lab 5 Punch Pass (value $100), you can bring 5 children to the lab at the same time or bring fewer children and spread out the fun over multiple visits.
This pass may only be applied to 1-hr Open Lab appointments and no other appointments/services. It has no cash value and does not expire. Current Participation Waivers must be on file for each person who enters the lab. Prepayment for a Punch Pass does not guarantee entry to lab without an appointment. Pass purchase price includes 7.25% sales tax.

With an Open Lab 10 Punch Pass (value $200) , you can bring 10 children to the lab at the same time or bring fewer children and spread out the fun over multiple visits.
This pass is valid exclusively for 1-hr Open Lab visits and cannot be applied to other appointments/services. Punch Pass Voucher has no cash value and does not expire.
Prepayment for a Punch Pass does not guarantee entry to lab without an appointment. Price includes tax at 7.25%
