Terms and Conditions - This agreement is for 12 consecutive months.
You Only Receive 1 treatment per month only from the options above. Any extra service requested will be charge at a regular price. This membership cannot be combined with another special.
Unused service cannot be rolled over to next month.
Payment :
We require a valid credit card to make your automatic monthly payment. A initial fee $20 will be added to set your membership on your first bill.
Please be aware that you will be charged automatically every billing cycle of 30-31 days.
WE ALSO ACCEPT PAYMENTS IN FULL (Cash or Credit Card) TO AVOID MONTHLY TRANSACTION FEE.
If your credit card payment bounces, a $22 declined transaction fee will be charged, plus 1.5% interest will be added to your monthly total of your membership every month till amount is paid in full.
If payment is not received after 60 days it will result on the automatic cancellation of your membership. A penalty fee of $50 will be added to your last bill.
If you choose to cancel your membership before the end of your 12 month term, we require 30 days in advanced to cancel your membership. You will be charged $50 cancellation fee and $70 charge for the "Add On" facial service resulting in the loss of the Free Upgrade and $4.99 transaction payment fee.
Cancelation and Tardiness Policy is in full effect.
We require at least 24 hours or more to cancel your appointment or reschedule. We have 10 minute grace period for tardiness after 15 minutes your appointment will be automatically cancelled, resulting in the loss of your 1 month service.
