This CLIENT SERVICES AGREEMENT ("Agreement"), effective as of today's date, is entered into by and between Lydia Pattison Consulting LTC ("LP"),having its principal place of business at 84 Percival Rd, Stanmore, NSW and you ("Client").

WHEREAS, LP is a company that provides its clients with Life Coaching Services, as further defined below; and

WHEREAS, Client is in need of Life Coaching Services and wishes to retain LP to perform such services;

NOW, THEREFORE, in mutual consideration of the promises set forth in this Agreement, the parties agree as follows:

SCOPE OF SERVICES

During the Term of this Agreement, LP shall provide the following Life Coaching Services for Client:

the services selected above hereinafter collectively referred to as “Life Coaching Services”.

Client acknowledges and understands: (1) that LP is not a licensed therapist, and Client cannot bill his/her insurance for services rendered pursuant to this Agreement; (2) LP does not provide diagnoses of any health or mental disorders; and (3) LP does not offer any guarantees of results in connection with the Life Coaching Services provided.

CLIENT RESPONSIBILITIES AND CANCELLATION POLICY

Client is responsible for timely communication with LP regarding scheduling.

While LP will do its best to accommodate schedule requests, Client must provide twenty-four (24) hour notice of cancellations and/or schedule change requests.

Client acknowledges and understands that there is no refund or reschedule for cancellations made with less than twenty-four (24) hours advance notice.

FEES AND PAYMENT

REFUNDS

There are no refunds for funds paid to LP.

TERM AND TERMINATION

PER SESSION

Clients on a Per Session plan agree to meet on the day(s) and time(s) agreed upon in Section 3.2.2, above.

Client agrees and understands that termination of a Per Session plan requires two weeks notice (in advance of the date of the final scheduled session); termination with less than two weeks notice will result in a forfeiture of those fees already paid.

Clients on a Per-Session plan agree that all Life Coaching Services must be used within 6 months of the first Life Coaching Service, or otherwise be forfeited.

EXIT SESSION

In the event of termination, it is LP’s preference that Client be available for a final Life Coaching Services session. Client and LP hereby agree to make all reasonable efforts to complete the final Life Coaching Services session.

CONFIDENTIALITY

Client hereby acknowledges and understands that no confidential relationship exists between LP and client. While LP is committed to treating Client’s information with the highest level of integrity, and will protect all of Client’s sensitive information, there is no legal protection given to Client’s and LP’s relationship by virtue of this Agreement or otherwise.

MISCELLANEOUS

SUBCONTRACTORS

Client hereby understands and acknowledges that LP may use its own employees or consultants or subcontractors to render the Life Coaching Services described herein.

NOTICES

All notices and other communications under this Agreement must be in writing and will be deemed to have been given if delivered personally, sent by electronic mail (with confirmation), mailed by certified mail, or delivered by an overnight delivery service (with confirmation) to the parties at the addresses listed below their respective signatures (or at such other address as a party may designate by like notice to the other parties).

GOVERNING LAW

This Agreement will be governed by and construed in accordance with the laws of the state of NSW, without regard to conflict-of-laws principles.

ENTIRE AGREEMENT

This Agreement constitutes the entire agreement and understanding of the parties with respect to the subject matter of this Agreement and supersedes all prior understandings and agreements, whether written or oral, among the parties with respect to such subject matter.

SEVERABILITY

If any provision of this Agreement is deemed to be invalid or unenforceable in any respect for any reason, the validity and enforceability of such provision in any other respect and of the remaining provisions of this Agreement will not be impaired in any way.

IN WITNESS WHEREOF, the parties have caused this Agreement to be executed and to become effective as of the day and year first indicated by payment for services.
