AGREEMENT TO HOLD HARMLESS, WAIVER AND ASSUMPTION OF RISK

I understand that attendance of dog obedience training is not without risk to myself, members of my family or guests who may attend, or my dog, because of the dogs to which I will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care.

I hereby waive and release Manners Matter Dog Training and Daycare, LLC, hereinafter referred as “training organization”, it’s employees, officers, members, and agents from any and all liability of any nature, for injury or damage which I or my dog may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of such damage or injury while attending any training session, or any other function, of the Training Organization, or while on the training grounds or surround area thereto.

In consideration of and as inducement to the acceptance of my application for training membership by this Training Organization, it’s employees, officers, members, and agents from any and all claims by any members of any family or any other person accompanying me to any training sessions or function to the Training Organization, or while on the grounds or the surrounding area thereto as a result of any action by any dog, including my own.

I understand that once a package is purchased it is non-refundable.

I understand there are no "make-ups" for missed sessions, classes or lessons.

I understand that scheduling classes, sessions, and lessons are not the responsibility of the training facility or their representatives

Your classes are 5 CONSECUTIVE classes. There are no make up classes. Please enroll in the first available class and consecutive classes once you have purchased your package.

**S.T.A.R. Puppy Group ClassEnrollees**

You will enroll for 5 CONSECUTIVE classes. There are no make up classes. Please enroll in the first available class once you have purchased your package. Puppies must attend all 5 classes plus your Group Class Prep course in order to earn the STAR Puppy certificate from AKC

**Pawsitively Trained Canine Coaching Private Lessons**
