This agreement is between Anna L De Salvo (“Coach”) and “Client”, the person participating in the appointment.
If an “Employer” is hiring the coach for an employee, all parties acknowledge that outcome-based information may be shared to assess if more coaching is needed.
All parties agree to the following: COACHING PRACTICE & RELATIONSHIP: Coaching is a relationship between the Coach and the Client to facilitate the process of personal, professional and/or spiritual growth and development.
The Client and Coach agree that the coaching relationship will be designed together. Coaching is for individuals who are emotionally and psychologically well and who want to make changes and move forward in their lives.
Coaching is not therapy, counseling, or personal training. The Client agrees to consult with their doctor before starting any new workout or nutrition program. By entering into this relationship, the Coach and Client acknowledge that the Client wants to make progress and/or change in his/her life and that progress and change happen at rates that are unique to each individual.
COACHING SESSION PROCEDURES:
Coaching sessions are conducted on Zoom or messages.
It is the Client’s responsibility to initiate the coaching session by arriving at the scheduled time.
The Coach will send the client prior to the scheduled time with the meeting link.
If the Client is more than 15 minutes late for the coaching session, the Coach will assume the session is canceled and the session fee will be forfeited.
Canceling and/or rescheduling a coaching session is easily done with appropriate notice. It can be done through the acuity scheduler or by contacting firstname.lastname@example.org
Please allow at least 24-hour notice to appointment changes. Appointments that are canceled or rescheduled within 24-hours will be charged $25 rescheduling fee.
The Coach has the right to terminate the coaching relationship at her discretion at any time and for any reason. If this occurs, the Coach will refund the party responsible for paying the fees for sessions that did not occur.

Payment Details: All fees are expressed in USD and are to be paid in advance of each coaching session, per the agreed-upon rates and dates. Payments may be made by credit card or PayPal.
If a package is paid in increments, the second payment must be made before the third session in a package is scheduled.
All packages don't expire, but the Coach recommends for best results - complete within 6 months. If the Client has chosen the payment plan option, the payment terms do not change if the Client decides to postpone sessions. If the unlikely event that the Coach postpones sessions, the payment terms will adjust accordingly to accommodate the party responsible for payment.
