You may expect anticipated benefits from the use of functional medicine in your care, but that no results can be guaranteed or assured.

If you are renting the FDA approved AlphaStim™ device for insomnia/mood balance, you understand the initial $30 fee covers cleaning and shipping cost to mail the device. The $49 per month payment is automatically deducted monthly until device is returned to Dr. Lara Salyer at 1005 16th Ave, Monroe WI 53566. You are responsible for payment to ship the device back to the clinic as well as the full purchase price of $840.00 USD if device is lost, stolen, or defective upon return, and this will be invoiced separately.

The Catalyst Studio™ mentorship for functional/holistic practitioners will provide business, educational and developmental advice and guidance in regards to personal and professional goals set forth. We will work together to develop a plan. There are no claims of guaranteed outcome.
There are no cancellations after 10 days from purchase date of the Catalyst Studio™ Mentorship, and you will be responsible for fulfillment of payment for the remainder of your 12 month commitment.
The Catalyst Advantage™ individual mentoring will help you identify blocks and gain momentum towards personalized goals set up at the first session.
