Welcome to Vytal Health! We are excited to have you as a member. Please read the following contract carefully and checkmark this section to indicate your understanding of the terms.

MEMBERSHIP TERMS:

Your membership with Vytal Health is for 12 (twelve) months starting on the day you join. You have the option to renew your membership at the end of 12 months. If you are in the FERTILITY PLAN, your membership with Vytal Health is for 6 (six) months starting on the day you join. We will send you reminders when your membership is about to expire.
You may change your doctor on the date your current membership expires. If you need to see an additional Vytal Health doctor, contact us and we will help you with the request. If you become dissatisfied with your doctor prior to your membership expiring, contact us and we will help you navigate as we want you to have an excellent working relationship with your doctor.
You may change plans at the time your current membership expires. You may also upgrade to a higher plan at any time. Upgrading your membership plan will be for a full 12 months from that point forward and a new contract will be sent to you to approve.

CANCELLATION:

Should you decide to cancel your membership with Vytal Health, simply notify your care concierge of your desire for cancellation. There is no cancellation fee.

PAYMENT:

You agree to keep your credit card on file with Vytal Health. We will charge your credit card for your membership each month on the day your membership began (unless you paid upfront for the year).

CHANGING/ADDING DOCTORS:

If you would like to change doctors, or if you would like to consult with another Vytal Health doctor outside your designated doctor, please contact us and we will help you arrange this.

ADDING ADDITIONAL TIME WITH YOUR DOCTOR:

In the event that you run out of plan appointment hours with your doctor, and you would like to add additional time prior to your membership plan expiring, you have the option to purchase a la carte hours to add to your membership. Please contact us to learn more.

LAB TEST AND SUPPLEMENT DISCOUNTS:

As a part of your membership, you will receive a discount on Vytal Health lab test kits and for supplements purchased through Vytal Health. The amount of your discount depends on the plan selected.

UNUSED APPOINTMENTS:

Appointment hours do not roll over at the end of a plan year. Any appointment hours not used during the plan year will be forfeited when the plan expires.

MISSED APPOINTMENTS & LATE CANCELLATIONS:

If you miss your initial (new) appointment, or cancel your initial (new) appointment without providing us with at least 24 hours notice of, you will be charged a missed appointment/late cancellation fee of $100.
Missed follow-up appointments or cancellations within 24 hours of an appointment will count as a visit for that plan year. You will not be charged an additional cancellation fee.
If your doctor has to cancel a visit for any reason, your cancelled appointment will not count as a visit for that plan year.

HOURS FOR CARE & COMMUNICATION:

Your care team is available for messaging during normal business hours (Monday through Friday, 8 am - 5 pm CST). Your doctor will respond to any messages you send within the portal typically within 24-48 hours.
If there is an urgent message related to your care at Vytal Health, we recommend you contact your care concierge for assistance during normal business hours.
For these plans, your Vytal Health doctor is not your primary care doctor. For any urgent matters after hours or on weekends, contact your primary care provider or proceed to Urgent Care or the Emergency Room, depending on the severity of the issue. We do not have after hours call coverage for these plans.

INSURANCE

Vytal Health does not accept health insurance or provide health insurance. We strongly encourage you to maintain health insurance during the term of this membership agreement to cover services that are not provided by Vytal Health.
Your membership fee does not cover any medical services outside of video visits with your Vytal Health doctor. Any services outside of Vytal Health, such as Emergency Room or Urgent Care visits, visits to specialists and tests done outside of Vytal Health lab test kits (such as radiology testing or blood draws in the hospital or clinic) are not included in the membership fee.
Vytal Health will not bill insurance carriers on your behalf but will provide, upon request, any necessary billing-related documents for you to submit to your insurance company directly for possible reimbursement as an out-of-network provider. We cannot guarantee that you will be reimbursed for any of our services, as this is at the sole discretion of your insurance company.
Vytal Health has opted out of participation in Medicare, however, some of our physicians are still enrolled in Medicare in their private practices. This means that we cannot bill Medicare for your care at Vytal Health and some of our physicians are not able to see Medicare patients. If you are a Medicare recipient, we will help guide you to the doctors who are able to see you. Please discuss this with your care concierge prior to joining as a member.

HSA/FSA PLANS:

Currently, membership fees covering visits with your doctor are an eligible medical expense that is payable or reimbursable using a tax-advantaged savings account such as a health savings account (“HSA”) or flexible spending arrangement (“FSA”). Additionally, all supplements and lab test kits purchased from Vytal Health are eligible for HSA/FSA reimbursement.

LIMITATIONS OF TELEMEDICINE:

On occasion, your Vytal Health doctor may recommend an in-person evaluation to gather additional medical information or for more immediate treatment. This is to provide the most appropriate medical treatment and is for your safety. Referrals to Urgent Care, Emergency Care, to specialists, or for other outside testing are not included in your membership.
Using telemedicine as your main connection to your Vytal Health doctor requires the appropriate equipment and internet signal. You agree to have a working computer or laptop with camera and audio capabilities as well as available internet connection for a video visit. At this time, video visits via cell phone and tablets are not reliable and are discouraged. Should your doctor be unable to connect with you via video at the time of your appointment, she/he will call you via telephone at the number you’ve provided to us to help troubleshoot the issue with you and/or recommend rescheduling your appointment to a later date. If an appointment is rescheduled at the doctor’s request, that visit will not be subtracted from your annual plan.
It is your responsibility to make sure that the environment in which you are having your video appointment is private. We do not recommend having video appointments in public places or areas where your privacy cannot be assured. Vytal Health doctors and team members will ALWAYS be in a private place when discussing your medical information with you, whether that is via telephone or video.

FUNCTIONAL NUTRITION PACKAGE

When purchasing the starter package, you will be charged a one time payment of $295. This will include one 60 minute initial consult and three 30 minute follow-ups. There is no expiration date on this package.
