Waiver:

In checking the box below I agree that Garage Pilates LLC is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at Garage Pilates may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns nor legal representatives will sue or make any other claims of any kind whatsoever against Garage Pilates LLC or its owners for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise. I understand that no exercise program is without inherent risks regardless of the care taken by a Pilates Instructor, personal trainer or group fitness coach and that my personal safety cannot be guaranteed. I realize that when participating in any exercises, particularly those that induce cardiovascular stress, there is a chance of serious injury (e.g. heart attack, stroke, or other cardiovascular accidents) or catastrophic incident (e.g., death, paralysis). Likewise, I know that engaging in muscular endurance, strength building, Pilates, and other fitness activities sometimes results in minor injuries (e.g., bruises, musculoskeletal strains and sprains), less frequently, more serious injuries (e.g., muscle tears, herniated disks, torn rotator cuffs) and rarely, catastrophic injury (e.g., death, paralysis).

I understand that it is my responsibility to 1) fully disclose any health issues or medications that are relevant to participation in a strenuous exercises program; 2 )cease exercise and report promptly any unusual feelings (e.g., chest discomfort, nausea, difficulty breathing, apparent injury) during the exercise program; and 3) clear my participation with my physician.

Should I choose to consume alcoholic beverages, I do not hold Garage Pilates or its owners liable in any way for what might happen after I leave the Pilates studio. I take complete responsibility to be in control of my consumption and will make safe decisions afterward as to not put myself or others in danger. Any action I take or experience I have while at Garage Pilates or after leaving is responsibility and I will not sue or enter into litigation with Garage Pilates or its owners regarding this. Garage Pilates is not to be held accountable for me choosing to drive and what happens as I do this.

Agreeing to this exercise program I acknowledge that my participation is completely voluntary. I understand that potential physical risks involved in Pilates and the exercise program and believe that the potential benefits outweigh those risks. I give consent to certain physical touching that may be necessary to ensure proper technique and body alignment. I understand that the achievement of your health and or fitness goals cannot be guaranteed. I have had a voice in planning and approving the activities selected for my exercise session. I have been able to ask questions regarding any concerns I might have, and have had those questions answered to my satisfaction. I am in good physical condition, have no impairment, which might prevent my participation in such activities, and have been advised to consult a physician prior to beginning this program. I have been advised to cease exercise immediately if I experience unusual discomfort and feel the need to stop. I give full permission for photos taken of myself and any minor I have legal guardianship/custody of to be utilized in printed or online form and request zero compensation for these photos. I am checking the box on this online waiver indicating I agree with all the above for both myself and the minor I have legal guardianship or custody of. Clients are able to book all their appointments online and submit payment at that time. Garage Pilates, LLC has a 24 hour cancellation/rescheduling policy. This is an industry standard. If an appointment is missed, canceled or changed with less than 24 hours notice, the client will be charged full price. The studio realizes that there are many things that come up in people’s day to day lives. While truly sympathetic, the studio cannot absorb the financial responsibility of last minute cancellations. Garage Pilates LLC does not double book appointment times and in fairness to all clients, this policy is in effect regardless of the reason for the cancellation. This policy is included in the liability waiver on the scheduling page. Refunds and rescheduling are available up to 24 hours before scheduled appointment time.
