CANCELLATIONS/RESCHEDULING:

★ If I am not able to make a scheduled appointment, I agree to cancel or reschedule the
appointment at least 24 hours in advance.

★ I understand that if I come in on a Gift Certificate and I no call no show I forfeit the certificate amount and validity. If I need to last minute reschedule on a Gift Certificate or Package I understand that I can do this once as long as it is within 4 weeks. Should I need to make another last minute change, I understand that I will not be able to reschedule using my Gift Certificate or Package.

★ I agree to pay the full session rate if I give 2 hours notice or less, or if I miss an appointment
without giving notice.

★ If, within 24 hours of my session, I develop an illness, or have a sudden unplanned health or personal emergency rendering me unable to make my appointment, I will inform Mama
Bear Massage LLC right away. Should I fail to inform Mama Bear Massage LLC, I understand that I will be charged the full session rate. I understand that should this occur more than 3 times in a 6 months time frame that I may be asked to leave a deposit when scheduling sessions in the future. This applies to Gift Certificates and Packages as well.

★ I understand that it is policy of Mama Bear Massage LLC to keep an active credit card on file. Should I opt to not put one in online, I understand that I will be asked to add one on file at the time of the session. I understand that the card on file will not be charged unless I have missed an appointment or cancelled with 24 hours or less.

★ I understand that I am still responsible for my appointment until I hear back from Shawna Patton or her assistant confirming that she has received my text or phone call requesting cancellation/rescheduling. Please do not email to make changes to your session or to cancel. Please do not send messages via social media, google, or any other way of communication to cancel or make changes.

ARRIVING ON TIME/SESSION LENGTH:

★ I understand that my session starts on time and ends on time. I understand that each session is booked with an additional 10 minutes to allow for discussion of session goals and any changes since my last session. Extending the session past the scheduled end time to account for a late arrival is not permissible.

★ I understand that in order for me to receive the best massage therapy possible, I know that I have
to communicate ANYTHING and everything, including my needs, preferences, requests or
feedback, at any time before, during, or after my massage. I take it upon myself to communicate
right away if there is anything distracting me or if I feel unwell or uncomfortable at any time
during the session so that she can make adjustments. I understand that my therapist wants my
HONEST feedback - positive or negative - and doesn’t take offense to it. I have read, understand, and agree to the above policies and information.
