Overview of contract conditions:
$150/month with autopay from a credit or debit card on file.
May be paused one time per year for up to 30 days.
30 days notice prior to your payment date is required for cancellation of membership
Membership and service sharing is prohibited.
CONTRACT
Monthly Unlimited Autopay contract is in force for 12 months from the Contract Start Date, with automatic renewal for each subsequent month, unless cancelled in writing by email, with notification 30 days in advance of the next billing date.
Cancellations
Cancellation of Contract may be made 30 days prior to next billing date.
Cancellations may be made in writing via email to infodragonflywellnesscenter@gmail.com 30 days in advance of the next billing date. (example: if you are scheduled to be charged February 14th and you want to cancel for the month of February, you must submit a cancellation request by January 14th.)
Agreement to Pay Recurring Fees: Client agrees to pay monthly recurring fees as follows:
via Client authorized automatic credit or debit card charges.
Payment in monthly increments by any means other than Client’s credit or debit card is not permitted.

I agree to purchase the Monthly Unlimited Autopay for $150 per month, as an automatic charge to my credit or debit card.
I hereby certify that I am the holder of the credit or debit card detailed below.
I understand that I will be notified if my payment payment fails to authorize for any reason, and that a $30 late fee will apply if I do not provide valid payment information within 10 calendar days of the notification date.
I understand that my service will be deactivated if my account becomes more than 30 calendar days late, and reactivation fees may apply if I choose to reactivate contract.
If auto payment fails two times during the contract, membership will be terminated if payment fails a third month. Reinstatement fees apply, and new membership will be available at the current monthly rate, with 3 months due up front.
