This Disclosure and Consent is intended to inform Client of the Coach-Client relationship. This coach is NOT a licensed Arizona professional counselor and the services provided should not be construed as psychotherapy under the meaning of Arizona Revised Statutes, Title 32, Chapter 33. While there are some similarities between coaching and psychotherapy, the goals, focus, methodologies, and level of professional responsibility associated with coaching are very different from those of psychotherapy, and it is important that you understand the differences between these two types of services. Psychotherapy is a health care service and is usually reimbursable through health insurance policies. That is not true for coaching. Coaching is NOT a resource for diagnosis, assessment or treatment of any medical problem or psychological disorder, nor is it intended as a substitute for seeking professional medical or psychological advice from an appropriate professional health care provider.
Both coaching and psychotherapy utilize knowledge of human behavior, applying interactive counseling techniques to address motivation and behavioral change. However, coaching focuses primarily on the development and implementation of strategies to reach client-identified goals of enhanced performance and personal satisfaction. Coaching may address specific personal projects, life balance, job performance and satisfaction, or general conditions in the client's life, business, or profession. Coaching utilizes personal strategic planning, values clarification, brainstorming, motivational counseling, and other counseling techniques without making clinical diagnoses of any medical or psychological condition.
The relationship between Coach and Client is specifically designed to avoid the power differentials that occur in the psychotherapy relationship. Except where disclosure is required by law, communications between Coach and Client will remain confidential. However, there will be shared experience and personal outreach brought into the coach-client relationship. The client sets the agenda and the coach asks the client to take risks and consider new approaches to life scenarios and objectives. Success will partially depend on the client's willingness to implement such guidance. This relationship is designed to be direct and challenging. You can count on your coach to be honest and straightforward, asking powerful questions and using challenging techniques to move you forward.
Client is expected to evaluate personal progress and fulfillment of expectations. At any time, Client can request that adaptive measures be implemented in the coaching process to address unique circumstances.
By agreeing below, Client represents that he or she understands the difference between psychotherapy and coaching and accepts that no diagnosis, assessment or treatment of any medical problem or psychological disorder will be provided in coaching services. In addition, Client agrees to hold Coach harmless from any claim of liability for alleged damages or injuries, whether personal, emotional, psychological, financial, or otherwise, associated with the information, ideas, suggestions, and techniques utilized by Coach. Client is strongly advised to seek professional advice as appropriate before making any financial or health related decisions.
