EFT SERVICES DISCLOSURE STATEMENT AND AGREEMENT

This document contains important information about my services, policies, practices and privacy/confidentiality.

EFT is a relatively new approach and whilst the body of scientific research is growing and providing some great results it is still considered experimental. Here is a link to some of the research http://www.eftuniverse.com/research-studies/eft-research

I am not a licensed physician or mental healthcare provider. EFT is not a substitute for the diagnosis or treatment of medical or mental health conditions by a licensed health care professional. If you have a disorder or medical condition that has been diagnosed by a licensed medical or mental health care professional or a condition that should be evaluated by a licensed health care professional, my services should only be used in conjunction with you obtaining/continuing that care.

I do not diagnose or treat medical or mental health disorders nor am I trained to do so. EFT attempts to optimise the body’s overall health and vitality, but is not to be used instead of appropriate care from a professional. EFT brings disturbed energy blocks back to a state of balance and harmony.

If you are uncomfortable being touched or with any of the procedures being used please tell me immediately and I will stop. The methods I use are gentle and non-invasive. If I feel the need to tap on you myself, I will always ask your permission first. (in clinic session only)

Confidentiality

With the exception of special situations described in the numbered list that follows, you have the right to absolute confidentiality of your therapy. I will always act so as to protect your privacy to the best of my ability. You may direct me to share information with whomever you choose, you may also request anyone you wish to attend a session with you.

I may occasionally find it helpful to consult other professionals about a client. During such consultation, I make every effort to avoid revealing the identity of the client. If you don’t object I will not tell you about these consultations unless I feel it is important to our work together.

The Exceptions to your right of confidentiality:

If I believe that you are in imminent danger of harming yourself
If I believe that you are threatening serious harm to another
In certain legal proceedings, a judge may order my testimony.

(In either of these cases I may legally break confidentiality and contact the police, a local crisis team or a family member or other intimates)

Billing and Payment

All Packages & bookings are payable up front at time of booking. If a session/appointment is cancelled or missed with less than 24 hours’ notice the session fee will still be payable in full unless medical emergency. If you are late to a session/appointment we will still finish on time.

Your acceptance by ticking the box indicates that you have read all the information in this document and understand it fully and you agreed to abide by the terms fully during our professional relationship.
