COACHING AGREEMENT AND DISCLOSURES PARTIES
This agreement between (Client) and Jessica
Farmer (“coach”, hereinafter referred to as “coach” or “Jessica”) and Jessica Farmer
Jessica agrees to coaching services, as designed by the coach. The deliverables of the
package agreement consists of:
24 individual one-on-one coaching sessions with Jessica. Each session will have
a duration of at least 50 minutes.
The sessions will be delivered through the conferencing software Zoom
(preferred) or by telephone. Use of Zoom requires the Client to download and
install the free Zoom app on their computer and/or phone.
Sessions conducted via Zoom can be audio-only or include Video. When using
Zoom the session can be recorded and an audio copy provided to the Client for
their future reference. Availability of these recordings is not guaranteed after this
agreement term expires.
In case of internet bottlenecks on either end, we will switch the session over to
phone, and will not have the possibility to record it.
The sessions are intended to be delivered at roughly weekly intervals over the
course of 6-8 weeks.
Any sessions unused after 8 months from the date of the first session shall be

Various written material and worksheets may be provided over the course of
the program. These are managed as PDF’s and will be either emailed as an
attachment or uploaded to a file wiehre the recordings are kept. The only
software required on your end is an internet browser.
The Program includes email, text, Voxer support and coaching for a period of 6
months, commencing with the date of the first session. Jessica shall respond to
Client's emails and messages within 48 hours.
Investment options are as follows:
A one time payment of $5000, received on or before the first date of service.
A automatic monthly payment of $850, received on or before the first date of service,
to be withdrawn on the same day each month, for 6 months.
REFUND and CANCELLATION POLICY
There are no refunds. If Client wishes to terminate the coaching contract, the automatic
monthly billing will be discontinued on the date of the request, after which no further
payments will be issued.
The program is strictly for educational purposes. Client accepts and agrees that Client is
100% responsible for their progress and results from the Program. Company makes no
representations, warranties or guarantees as to results, verbally or in writing. Client
understands that because of the nature of the program, the results experienced by each
client may vary significantly.
Client acknowledges and agrees that coaching is a comprehensive process that may
explore different areas of his/her life, including work, finances, health and relationships,
but it is ultimately the Client’s decision how he/she incorporates coaching into each
aspect of life.
The main benefits from this program will come from the coaching sessions. Client will
maximize these benefits by:
Showing up to the session on time
Being situated in a quiet distraction-free environment with good cell or internet
connectivity. Jessica won't coach if client is driving. Please avoid being in a
public area (such as a coffee shop) where Client can be overheard.
Upon starting calls have phone and desktop in do not disturb mode.
Having read any preparatory material provided prior to the session and worked
through assigned worksheets.
Having paper and pen with which to take notes.
The homework and preparation is purely optional - you don't have to do homework,
take notes, or have any sort of agenda. All you really have to do is show up to the
Zoom call within 10 minutes of the scheduled start time and I will coach you to the
absolute best of my ability.
The coach to client relationship is special and scared. It is not like that of a friend or a
therapist or counselor. Unlike a friend, a coach is a neutral and not subject to gaining
favor by agreeing or “taking sides”. Unlike a therapist or counselor, our focus will be
heavily oriented towards your future rather than your past. It is the job of the coach to
unconditionally love you and see you as the highest and best version of you. Jessica
coaches as a guide and teacher, focusing on the cause of your problems rather than the
symptoms. This facilitates the client’s increase in awareness of the power of
their thinking and more capable of self-coaching in between visits.
Client should affirm the following statements:
I the Client understand that the coaching provided by Jessica is NOT licensed health
care, and as such is most likely not reimbursable through health insurance policies.
I understand and agree that I am fully responsible for my well-being during my
coaching sessions and subsequently, including my choices and decisions.
I recognize that coaching requires emotional, physical, and mental exertion and
behavioral experimentation on the client’s part. I fully acknowledge any risk of
mental or emotional injury and take full responsibility.
All information about the coach/client relationship will remain strictly confidential except
where decreed by law (ie. where the court might issue a subpoena for the file or
Exceptions to confidentiality of course relate to circumstances such as intent to
seriously harm someone, child abuse, threats of suicide etc. Otherwise, all remaining
information is kept confidential.
If you wish for Jessica as your coach to speak to someone outside of our interactions,
you need to provide written permission (via written letter or email) to do so.
SESSION CANCELLATION & RESCHEDULING POLICY
Client may cancel a scheduled coaching session by sending an email to
firstname.lastname@example.org at least 24 hours prior to the scheduled session.
Sessions that are not canceled and rescheduled according to this policy may be forfeit.
If Client is more than 15 minutes late to a scheduled session, the session shall be
