In order to receive the services provided by the IV Club, LLC (the “Company”) you must review carefully the below Limitation and Waiver of Liability and make sure you fully understand its contents before signing. I understand that the Company cannot provide its services until I do so.

I understand that these services cannot treat or cure any medical condition. I understand that the services offered by the Company are being carried out pursuant to the standing orders of a physician, and I understand I will not be seen by the responsible physician. It is my responsibility to consult my own medical provider regarding any medical or health care matters. I agree that I am responsible for my health care. All medical conditions should be treated through a direct visit with a provider, and the Company takes no responsibility for any medical conditions. I understand that the Company is not licensed by any state or federal agency which oversees or supervises the provision of health care services.

I represent that at the time of signing I am in good health and without any medical condition that would exclude patient from receiving Company services such as uncontrolled diabetes, heart failure or cardiomyopathy, acute or chronic kidney disease, or any other acute, chronic, or severe medical condition. I also represent that I do not suffer from any allergy or intolerance to the following medications; classes of medications or medical supplies: IV fluids, Toradol/NSAIDS, Zofran/Ondensetron, or latex. I understand that I should not seek the Company’s services if I have any of the listed conditions.

I agree that I have read and reviewed the information about these services contained both herein and on the Company’s website, and that I agree to abide by all terms and conditions outlined. I fully understand and agree to comply with instructions. This agreement is in effect for all treatments and will not expire unless requested by either party. I also promise I have not been induced to seek these services, and I am assuming full responsibility for myself and the use of these services.

I represent that at the time of signing, I am not under the influence of any intoxicants and that I am sober and otherwise competent to enter into a binding legal agreement, such as the one set down herein.

I agree to the following LIMITATION AND WAIVER OF LIABILITY. I AGREE THAT IV CLUB, LLC AND ITS EMPLOYEES SHALL NOT BE LIABLE FOR ANY DAMAGE RESULITING FROM MY USE OF ITS SERVICES. THIS DISCLAIMER AND LIMIT OF LIABILITY COVERS CLAIMS BASED ON WARRANTY, CONTRACT, TORT, STRICT LIABILITY, MEDICAL MALPRACTICE, AND ANY OTHER LEAGAL THEORY AND ALL LOSSES INCLUDING, WITHOUT LIMITATION, DIRECT OR INDIRECT, SPECIAL, INCIDENTAL, CONSEQUENTIAL, EXEMPLARY, AND PUNITIVE DAMAGES, PERSONAL INJURY/ WRONGFUL DEATH, LOST PROFITS, OR DAMAGES RESULTING FROM USE OF THE IV CLUB, LLC’S SERVICES.By signing below, I affirm that I have read, understand, and agree to the contents of this Limitation and Waiver of Liability.
