By checking the box, I understand that after 7 days scheduling my appointment, my purchase is non-refundable. I understand that it is my responsibility to be on time for me appointment, and in the case I am later than 5 minutes after my appointment time, I forfeit my session and payment.

I understand purchases of gift certificates are to be scheduled before start of the next calendar year, after which, the code will expire.
