LIABILITY WAIVER

I, the undersigned as parent/legal guardian, authorize said child’s participation in SFT Athletics training program. In and for consideration of my child’s participation in the program, I hereby waive and release for myself and my heirs, any and all rights or claims I may have against SFT Athletics, any affiliates or subdivisions of SFT Athletics. I hereby, agree that I will not hold SFT Athletics nor its staff/employees responsible for any loss, damages, or personal injuries that he/she may receive as a result of participation. This waiver of liability expressly includes training and camp activities, or while in, on or upon the premises whereby the activity is being conducted and transportation to and from, or in connection with, said training. I also agree to follow all instructions and procedures in order to maintain a maximum level of safety. I also understand that I should make sure my child is covered in the event of a serious accident. I also give my permission for any emergency medical care or treatment by a physician, surgeon, hospital or medical care facility that may be required, and accept the responsibility for the cost.

I, the undersigned as parent/legal guardian, agrees that:

The risk to have contact with individuals, who have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical conditions, diseases, or maladies does exist, and it is impossible to eliminate the risk that I could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease.

PHOTO/VIDEO WAIVER AND RELEASE FORM

I, the undersigned as parent/legal guardian, hereby give my permission to SFT Athletics Group LLC, to use photographs, voice recordings, or video taken of my child during the training programs associated with SFT Athletics in any manner to help promote the training activities as determined in the sole discretion of the SFT Athletics group. Such use could include publications, media releases, public announcements, electronic or otherwise, and on SFT Athletics websites or social media pages. I agree that neither I, nor my child, will receive any compensation if such image appears in any of the manners listed above or other manner that SFT Athletics deems appropriate. I agree that such image is the property of SFT Athletics Group LLC.
