Monthly Membership Terms and Conditions

Your membership entitles you to prepaid discounted service(s) as stated in the description of the membership option of your choosing. You may receive unlimited additional services during the month at 20% off of the non-member price. You receive 10% off all retail products sold at Alchemy Holistic Wellness, as well as any packages. Your Membership is valid only at the Algonquin, IL location and for remote appointments. Membership will auto-draft monthly, unless otherwise agreed upon. Accounts can be placed on hold for up to 3 months per membership calendar year. Either party may cancel the membership at any time by giving written notice to the other party. Your membership and pre-paid session(s) can be shared with two additional family members. If you have a Paid in Full membership, you must use all of your prepaid session(s) prior to the expiration of the membership (365 days from start of membership). You understand that if you are not able to come during a particular month, you still will be charged, and your pre-paid sessions will roll-over to the next month. A minimum of 3 months consecutive payments required. Unused sessions will expire after 90 days. Member benefits may not be combined with any other promotional offer or promotional gift card.

Your membership is set up month to month and auto renews until you cancel. You understand that you must give written notice to cancel your membership, 14 days prior to next payment. You will have 90 days after this payment to use any remaining accumulated sessions, after which all accumulated sessions, will be forfeited from your account and your membership will be considered terminated. There will be no refund of any kind on terminated sessions. Members will be given at least 30 days' notice in writing of any changes, which include: any increase in membership fee, or change in date of automatic withdrawal. If you have a Paid in Advance membership you understand you will not be able to be refunded any money for any portion of your unused sessions.

You must agree to a minimum of three consecutive months.

Alchemy Holistic Wellness may cancel your membership at any time and all pre-paid sessions will be considered forfeited by you if:

You abuse your membership due to misuse of appointments such as excessive cancellations or no-shows.

If you have insufficient funds in your account for your monthly membership fee three consecutive months in a row from the provided Credit/Debit Card. Use of accumulated pre-paid sessions will not be allowed until a new credit card is updated in our system.

Inappropriate behavior from the clients or Therapists will not be tolerated in any manner. Please notify the Manager immediately for appropriate action.

We reserve the right to terminate or deny re-enrollment for an undetermined amount of time for not following any of our Policies & Procedures.
