Memberships

12 Month Therapeutic Massage Membership Agreement

This agreement is between The Art Of Chi - Mobile Massage Therapy (referred to as Kianis Henderson) and the joining member.

Term:

This membership is valid for a period of 12 months from the effective date of enrollment. Payments will be automatically debited from the Member’s provided payment method on the first day of every month following the signing up.


Membership Benefits:


1. Clients will be allotted a massage credit each month when the payment is processed.

2. Additional massages beyond the allotted credit will be available at a discounted rate.


Household Membership:


1. This membership covers all individuals residing at the same address.

2. Each member of the household is entitled to the benefits outlined in the “Membership Benefits” section.


Membership Fee:

The monthly membership fee is $100. Payments will be automatically debited from the Member’s provided payment method on a monthly basis.


Cancellation:


1. The Member may cancel this membership after a minimum of three (3) consecutive months from the enrollment date.

2. Cancellation requests must be submitted in writing [via email or physical mail].

3. If cancellation is requested before the completion of the three-month period, a cancellation fee will apply.


Renewal:

Upon the expiration of the 12-month term, this membership will automatically renew on a month-to-month basis unless the Member provides written notice of cancellation at least 30 days before the end of the 12-month term.


Termination by Provider:

The Provider reserves the right to terminate this membership at any time for reasons including but not limited to violation of policies or inappropriate behavior.


Miscellaneous:


1. This Agreement is non-transferable.

2. Changes to this Agreement require written consent from both parties.

3. Any outstanding fees upon cancellation are due immediately.
