PLEASE READ THIS CAREFULLY. It affects any rights you may have while your dog(s) is participating in our private training or group class. It also states your responsibilities regarding the fees and expectations regarding the K-9 obedience programs offered by SHIELD K9.

I, (participant/owner of the dog) hereby agree to the following covenants described below regarding the services offered by SHIELD K9. I further release, waive, discharge and covenant not to sue SHIELD K9, Hazhaar Othman and any of the officers, servants, agents and employees of the above-mentioned entities (hereinafter referred to as RELEASEES) for any liability, claim and/or cause of action arising out of or related to any loss, damage or injury, including death, that occurs as a result of my participation in the below-described activities.

Tuition for this class / package is due in full at the beginning of the course.
By signing this agreement, you are giving your consent for your dog to take part in private classes or group classes as determined by the trainer and also to play and train with other dogs as deemed safe and beneficial by the trainer.
Participating dogs are to be current on their vaccinations. We require a current DAPP vaccine, Bordetella and Rabies vaccine (if the dog is old enough at the time of class). By signing this agreement, you certify that your dog is current on his/her vaccinations. Each participating owner is responsible for preventing unwanted pregnancies if his/her dog is not neutered or spayed and/or he/she is in heat or another participant’s dog is in heat.
I understand that vaccines are not 100% fool proof and that there is a minimal risk that my dog may contract a contagious virus/disease including kennel cough. I agree that should this occur I am responsible for my own pet’s care and medical attention.
I understand and acknowledge that while Shield K9 makes every effort to safely supervise and contain dogs in their care, accidents including but not limited to injury, loss and death may occur and I further release Shield K9 and all Releasees and associates from any and all responsibility.
I agree to fully disclose any pre-existing injuries or medical conditions or behavioural issues my dog has prior to entering into any service offered by Releasees.
In the event that my dog injures another dog or person, I accept medical and legal responsibility for my dog’s actions and release the Releasees from any liability.
The Releasees hereby has permission to use any photos taken of you and/or your dog during class for advertising purposes. Names of owner(s) and/or participant will not be printed or released.
