I understand that there are expiration dates for all packages and that I have the following time periods to use all bookings within the following packages: an intensive 5-pack within 1-year of purchase, all bookings in an intensive package or a 10 touch in package within a 6 month period, all bookings in an intensive duo within a 3 month period, and all bookings in a whole person care package or whole person care package plus+ within a 2 month period. I understand that by booking, paying for, and scheduling these appointments, that any unused appointments do not carry over past these specific time durations per each package and that I will not be refunded for cancellations or missed appointments.

I understand that I must schedule a monthly intensive as an initial appointment (first appointment) and fill out an intake form and agree to all terms and conditions. I understand that monthly intensives are limited to one appointment per month unless specified by my care plan. I understand that a "touch in" cannot act as an initial appointment and that "touch in" cannot be scheduled back-to-back or in the same day for the same patient.
