RECURRING PAYMENT AUTHORIZATION AGREEMENT: I authorize JEWEL ATHLETIC SCHOLASTIC INSTITUTE, LLC, JASI Jayhawks, JASI of Learning to initiate an electronic debit either by credit card or e-check against my bank account to the terms below. Select a form of payment, credit card, or e-check, and complete the other requested information that is required. Upon approval, I will automatically be billed for the amount indicated, and my total charges will appear on my monthly credit card/ bank statement.

AGREEMENT CANCELLATION: All membership agreements are set up as recurring payments according to the terms the customer agrees to and will require your electronic signature. Once your term is completed, the customer may cancel the auto-renew membership through their account. Cancellation must be done 24 hours before the renewal sales date. If the membership agreement is not canceled before the date of the sale, the current agreement will automatically be renewed, and payment is non-refundable.

LATE FEES: A $35 late fee will be charged to the customer for any failed attempts to withdraw funds from the customer's account after the 7-day grace period. If the balance remains after 8 days from the billing date, the late fee will remain on account and services will be suspended until there is a zero balance on account.

ALL payments are non-refundable for all services, memberships, teams, or programs.

JEWEL ATHLETIC SCHOLASTIC INSTITUTE, LLC, JASI Jayhawks, JASI of Learning reserves the right to change or terminate service options, prices, dues, fees, programs, classes, and events at any time.

ACCEPTED METHODS OF PAYMENTS: Money Orders, Cash (exact amount), Debit/ Bank Card with Visa or MasterCard logo, Credit Cards.

RENDERING SERVICE: Customers must have a debit/bank card or checking account on file or services will not be rendered.

FEES: JASI will collect fees prior to rendering services. In the event of a change in fees, JEWEL ATHLETIC SCHOLASTIC INSTITUTE, LLC, JASI Jayhawks, JASI of Learning may provide any notice required by law by regular mail or by email to the address associated with the customer.

ACADEMIC REQUIREMENTS: Students are encouraged to maintain a 75% average or satisfactory and above in all classes. Student athletes in GT or advanced classes are encouraged to maintain a 70% average and above in all classes. Students or parents are required to submit updated copies of the current progress report or report card to their designated coach via e-mail or in person prior to training sessions. If a student-athlete falls below a 70% grade point average during the six-nine week grading period, our discipline procedures from our student-athlete/ parent agreement will be enforced.

1-on-1 Training / BB Group Training/ Performance Training:

Student-athletes must be within a specified age range to receive services.

ARRIVING LATE: There is a 10-minute grace period before the session will be considered a no-show/ missed session. 11 minutes late, services will not be rendered and the customer will be charged 100% for that session.

CANCELING SESSION: a scheduled session must be canceled 24 hours before the initial scheduled session or it will result in a no-show/missed session. Please note that we are not guaranteeing availability for another session if the client is needing to reschedule.

MAKE-UP SESSIONS: occur only when JASI Staff is sick and unable to attend scheduled sessions, weather (if outdoors), or location conflict. JASI will inform immediately if there are any changes to the scheduled session by email, text, or phone.

PAYMENT OPTIONS: per session fee, per monthly fee, or membership fee. All payments must be remitted prior to receiving service to the terms the customer agrees.

Scholarships are offered, but not guaranteed. Refer to and complete the “Getting To Know You Form” (when available).
