-I am aware that I must be a current Member of Your Life Fitness Centre to participate in any of these services, excluding body scans, which can be booked by non-members. If you are not yet a member, please give us a call on 6581 0059 or see us at reception during staffed hours.

-Cancellation policy: If change of session is requested outside of 12 hours, this is to be re-booked as a "catch up" session within 2 weeks. If change of session is requested within 12 hours then the session will be charged & cannot be caught up.

-I am aware that the PT sessions I have purchased are non-refundable & non-transferable.

-I am aware that I am voluntarily participating in Personal Training which involves progressive physical activity and may include nutritional advice. I acknowledge the strenuous nature of Personal Training and the potential to cause risk, injury and even death. I have obtained a Doctor’s clearance certificate if I am unsure about my health prior to commencing Personal Training. I acknowledge that I may stop training at any point during my training sessions. By indicating that I have read this document I assume all risk for my health and well-being and any resultant injury or mishap that may affect my well-being or health in any way and hold all of the responsibility away from Your Life Fitness Centre, the instructor, facility, or persons involved with the Personal Training.

Smart Start & Level Up - One time purchase - I am aware that I can only purchase one Smart Start program & one Level Up program per person. If you want to continue with one-on-one training after these program, please purchase one of our Personal Training options.

Level Up - Prerequisite - I am aware that I can only participate in a Level Up program if I have recently completed a Smart Strat Training or Results Guaranteed program.
