Membership Charge Authorization

I authorize Gadd Group Health Centre to deduct my monthly dues from my credit/debit card. The monthly deductions will begin on the date of sign-up and continue being charged on the same date every month. I understand charges will continue month-to-month until I choose to cancel or freeze my membership. Other membership options are available after the initial trial period and will be discussed towards the end of your trial.
