Ignite PRP LLC Center Electronic Client Agreement

This Client Agreement ("Agreement") is entered into by and between Ignite PRP LLC, hereinafter referred to as "the Center," and the undersigned client, hereinafter referred to as "the Client." This Agreement outlines the terms and conditions governing the Client's use of the Center's facilities, services, and therapies.

1. Services Provided

The Center agrees to provide the Client with access to its facilities, wellness therapies, equipment, and services as described in the selected membership package or service agreement, specifically the [Membership Type]: ___________________________. The specific details of the services and membership options are outlined in separate documents, including but not limited to membership contracts, pricing sheets, and waiver forms.

2. Assumption of Risks

The Client understands and acknowledges that the use of the Center's facilities, therapies, and equipment carries inherent risks, including but not limited to physical injury, discomfort, and potential adverse reactions. The Client willingly assumes all such risks and agrees to use the Center's services, including wellness therapies such as cold plunge, infrared saunas, and other modalities, in a safe and responsible manner.

3. Health and Fitness

The Client certifies that they are physically and medically fit to participate in the wellness therapies and activities offered by the Center. The Client has not been advised by a qualified medical professional to refrain from participating in these activities.

4. Waiver and Release of Claims

In consideration of being allowed to participate in the Center's services and activities, the Client, on behalf of themselves, their heirs, executors, administrators, and assigns, hereby waives, releases, and forever discharges the Center, its owners, employees, agents, representatives, and affiliates (collectively referred to as "the Released Parties") from any and all claims, liabilities, demands, actions, or causes of action, whether at law or in equity, arising out of or relating to any physical injury, illness, or harm that may occur while participating in the Center's services, including but not limited to the use of wellness therapies and equipment. This release includes claims for the negligence of the Released Parties.

5. Indemnification

The Client agrees to indemnify and hold harmless the Center and the Released Parties from any and all claims, demands, losses, liabilities, costs, and expenses (including attorney's fees) that may arise as a result of the Client's use of the Center's services and facilities.

6. VIP Recovery Package - 6 Month Contract

The Client acknowledges and agrees to participate in the VIP Recovery Package, which is a 6-month commitment. This agreement cannot be canceled during the 6-month period.

7. Cancellation Policy

The Client acknowledges and agrees to provide the Center with a minimum of 25 days' advance written notice, delivered by email to Jsch6274@gmail.com, for the cancellation of their recurring payment and membership. Failure to provide timely notice may result in additional charges.

8. Electronic Funds Transfer

The Client acknowledges and agrees that all monthly membership fees, including the VIP Recovery Package, will be automatically deducted from their designated debit or credit card on a recurring basis on the (enter Day) ___________________ of every month/year depending on the membership chosen.

9. Paid in Full - VIP Founders Membership

The Client acknowledges that the VIP Founders Membership is a non-refundable payment option. The Client also understands that with proper documentation, their membership may be paused for a specified duration.

10. Governing Law and Jurisdiction

This Agreement shall be governed by and construed in accordance with the laws of the state of Michigan, without regard to its conflict of laws principles. Any legal actions or proceedings related to this Agreement shall take place in the state and federal courts located within the state of Michigan.

11. Entire Agreement

This Agreement constitutes the entire agreement between the Center, superseding any prior agreements or understandings, whether written or oral.
