Please email me your paperwork (for initial consultations please make sure you return the consultation paperwork) as well as any lab work, audiometric exam, MRI, CT scan or any other diagnostic that I need for this consultation. If you do not have these or you cannot get them for this consultation then we will do without and if need be we will request them at a later date. All hearing loss consultations need an audiometric test even if its for tinnitus please! :)
