Step 2. Applicant completes CPHQ online (link to CPHQ will be sent to email address entered below).

Step 3. Applicant completes Q-Global psychological testing online (proctored via encrypted audio/video link).

*** Q-Global testing must be completed prior to Step 4 ***

Step 4. Applicant schedules and completes face-to-face interview with the doctor (via encrypted video).
