• I have not engaged Cedric Bertelli for psychotherapy, bodywork or to treat a mental illness.
• I give Cedric Bertelli permission to assist me in investigating any accumulated stress I may have.
• The service provided is alternative or complementary to healing arts services licensed by the state.
• The services to be provided are not licensed by the state.
