The FUH/FUM program is for patients recently discharged from a psychiatric hospital or ED. Select a time window (8-10am, 3-5pm) on the next available date and a provider will contact the patient during that time frame.
Eligible Health Plans: CDPHP | Healthfirst | MVP | UHC Optum NY-Community Plan
Please submit a general outpatient referral for patients with all other health plans. Patients discharged from general medical services are ineligible.
