Customer ID instead of name fields
In order to further minimize collection of Personally Identifiable Information (PII), appointments are booked using a personalized and unique Customer ID in place of names.
Please create your Customer ID using the formula below and enter it in BOTH Customer ID fields. The form will not submit without both fields filled out.
FIRST INITIAL + LAST INITIAL + LAST FOUR DIGITS OF PHONE NUMBER
Example: John Doe, phone number +49123456789
Customer ID: JD6789
Garrison Security Office appointments are not currently available through the online scheduler. Please contact the Security Office to arrange an appointment.
