All appointments are set as Telehealth by default, but in-person visits are available if preferred. You can choose the in-person option through the intake form sent 3 days before your appointment.

Deposit: A $10 deposit is required to secure your appointment. This will be fully refunded after the visit, applied to your co-pay, or refunded if canceled with more than 48 hours' notice.
