All appointments are set as Telehealth by default, but in-person visits are available if preferred. You can choose the in-person option through the intake form sent 3 days before your appointment refunded if canceled with more than 48 hours notice.

Deposit: A $25 deposit is required. This will be fully refunded upon the visit, applied to your co-pay, or refunded.
