YOU MUST HAVE THE FOLLOWING ITEMS TO BEGIN
THE APPLICATION PROCESS:
FAILURE TO SUBMIT THESE ITEMS WILL DELAY THE PROCESSING OF YOUR APPLICATION.
All Clients must provide the following:
- Copy of parent/guardian photo identification in the form of driver license or other government issued identification
- Proof of residency in the form of a valid Florida driver license or current utility bill (within the last 12 months showing a service address)
- Proof of age for all children in the household (copy of official birth certificate or immunization record)
- Proof of citizenship, non-citizen national, or qualified alien for children receiving services- Examples include: (Birth Cert., Passport) Please note: Families with children born in Florida can visit the Florida Health Department to request a Birth Cert (fee) http://www.floridahealth.gov/certificates/certificates/birth/index.html . For children not born in the
state of Florida, you can go to https://www.cdc.gov/nchs/w2w/index.htm for more information on how
to obtain vital records. - Proof of Guardianship for all children in the household- Examples include: (Birth Cert. showing guardian’s name, court order or other legal document)
- Proof of all household income. See income types listed below and be prepared to bring documentation that is applicable to your status.
- If enrolled in school, an official school letter showing enrollment status. Enrollment status must show Full Time status if unemployed.
Employment Income
- Last 4 consecutive pay stubs if paid weekly OR
- Last 2 consecutive pay stubs if paid bi-weekly OR
- Last 2 consecutive pay stubs if paid semi-monthly OR
- Most recent pay stubs if paid monthly OR
- Employment verification form provided by ECS completely filled out by employer with verifiable signatures OR
- A letter on company letterhead with verifiable signature (from payroll, HR, or company manager) including hire date, rate of pay and hours working per week OR
- If self-employed, your most recent tax return
Child Support
* Please note that child support includes any amount of money given/received to help with the cost of care for a child
If received:
- Court order and proof of receipt for the last 4 weeks or 1 months OR
- If court ordered, printout through child support enforcement website www.myfloridacounty.com OR
- If payment is not court ordered, written and dated statement from absent parent indicating payment dates and payment amounts for the last 4 weeks or 1 months OR
- smiOne debit card statement
If paid out:
- Court order and pay stubs showing child support is being deducted from pay OR
- Copies of checks/money orders showing child support is being paid out
Alimony
If received:
- Court order and statement from payee that payments are being received as stipulated. Statement must be signed and dated by former spouse and must include former spouses address and phone number OR
- Court order and copies of payments for the last 4 weeks or 1 months
If paid out:
- Court order and proof of payment for the last 4 weeks or 1 months OR
- Court order and copy of pay stubs showing alimony is being paid out
SSI/SSA
- Current yearly award letter showing amount being received (a benefit letter can be obtained online by creating an account at www.ssa.gov OR
- Written letter from SSA showing current year of monthly gross benefits before deductions
*If SSI/SSA is being received by disabled parent, the current SSI/SSA award letter of the disabled parent will be required. If disabled parent is not receiving SSI/SSA, a licensed physician’s statement stating permanent disability stating that parent’s disability requires childcare assistance
Retirement Benefits or Veterans Benefits
- Current yearly award letter showing the amount received
- Written statement from source stating the amount that is given and how often
- A valid referral and TANF letter for TCA clients
