With this service, you will be able to have your raw DNA file from either of these companies evaluated by our personalized genetic tools that distill your genetic data into simple reports that enable you to make informed lifestyle and nutritional recommendations that are customized to your unique genetic profile and wellness goals. We will not sell, share, or rent a patient's 23andMe® or Ancestry® data files to third-parties. This service offers even deeper personalization with an optional, short questionnaire to help refine your lifestyle and supplement recommendations, with the guidance of your healthcare provider. You will receive customized nutrigenomic reports that are backed by science and serve as your roadmap to more informed health and wellness decisions. In addition to your customized nutrigenomic report you will also receive a customized 30 day nutritional program based on your results to better support your nutrition from a genetic level. This service is educational only in nature and is not covered by or billable to insurance. You do not have to be a patient to purchase this service since it is educational in nature and any findings from this report should be discussed with an experienced health care professional familiar with nutrigenomics (like Dr. Kourtney for example) before taking action on your report. TO BE CLEAR YOU MUST HAVE A RAW DNA FILE FROM ANCESTRY OR 23 AND ME TO USE THIS PACKAGE. ONCE THE PACKAGE IS PURCHASED YOU HAVE 45 DAYS TO SUBMIT YOUR RAW DNA FILE FOR YOUR REPORT.

This package includes 2 groups visits. In the first group the doctor will review different types of monitors, how they work, walk you though best practices of daily use and the best way to interact with the CGM to get quality and actionable data. The 2nd group is focused on report interpretation and data integration and each participant will have the opportunity to review their personalized data for actionable insights and learn how they can integrate this type of monitoring going forward in their health and wellness. THESE GROUP IS NOT BILLABLE OR REIMBURSABLE BY INSURANCE. All groups must be completed within 60 days of booking your package.
Asynchronous Case Review Report$649.00
The Asynchronous Case Review Report includes a review of your medical history and previous medical records (up to 2 years) through a functional lens. You will also complete an energetics and doshic evaluation as part of this process. Based on those results and your records a case report is prepared as to where your health appears to currently stand from a functional medicine perspective and recommendations are given as to where to start holistically with recovering your health. This is done asynchronously which means there is no one on one time with the provider with this option however it is the provider who reviews and prepares the report. Timeline from upload of records to case report delivery: 10 days This means in 10 days you could have a detailed functional assessment of your case and suggestions as to options to explore further. For many this can be a great way to start on their functional medicine journey if they have already seen other providers and if they have already had bloodwork and/or testing performed.
