Intake Appointment

Thank you for scheduling your intake appointment. Please schedule your first appointment only. All sessions are currently being offered online through a confidential telemedicine portal. During your intake assessment your counselor will help you coordinate all follow-up sessions at the most convenient time for you. After scheduling an intake appointment you will receive an email with the Intake evaluation forms. Please take your time and accurately complete the behavioral health questionnaire and sign all the attached consent forms and submit them. This form will help the therapist assess your needs and help you devise a treatment plan to successfully achieve your therapeutic goals.

In the form you will be asked to register your means of payment before your session starts. You must enter your credit or debit card to register for intake. If you have active full medical insurance coverage you will not be charged. If you successfully submit all the required documentation the clinician will contact you by phone around the scheduled time of your intake appointment. The intake session might not be at the exact time it was scheduled but will be as close as possible. Please be patient and allow at least a two hour window.

Documentation Request

To request needed documentation for free at no cost including progress reports and or attendance reports please do so no less than one week in advance. Please schedule your documentation request at the link below and please accurately answer all the required questions. You will receive your progress report within one week.

Expedited Documentation Request

To expedite a request for documentation please do so at the link below. All documentation requests should be made no less than one week in advance. If you missed the one week requirement please make an expedited request for documentation. There will be a $50.00 fee to produce an expedited letter. There is no guarantee for an immediate letter but we will try to have it produced within 48 hours.

Clients are expected to attend all scheduled sessions. If you miss a session you will be expected to pay a minimum fee for the missed session. If you can not attend a session please formally request to be excused from a scheduled session no less than 12 hours in advance. Please do so at the link below.
