Before this appointment, you will receive a Nutritional Assessment Questionnaire. This will consist of a three-day food journal. Write down everything you eat or drink for three full days - and I do mean EVERYTHING. Be honest! This will only help you! I will look over your journal before your appointment, so please plan accordingly and have it submitted on time. We will discuss quantity and quality of foods and beverages, so please list approximate amounts/brand you're consuming.

Plan on a little more time for this appointment. We will be doing a hands-on Functional Evaluation. Please wear comfortable clothes as you will be standing, sitting, and lying down as part of the appointment. Pants or shorts are preferable to skirts or dresses, if possible. Please do not eat or drink anything within 30 minutes of this appointment (food, gum, candy, beverages), except for water.

This is for anyone who has already seen/spoken with me at least once. I look forward to seeing/talking to you again!
