• The following does not apply to TMS or ketamine initial consultations.
• A deposit is required and will be fully refunded upon visit, applied to co-pay, or refunded with more than 48 hours notice.
• If the appointment is a no show, without prior cancellation notice, an additional $150 will be charged to the credit card on file.
• If the appointment is cancelled the same day, an additional $75 will be charged to the credit card on file.
• To cancel or reschedule your appointment, there is a link in your appointment confirmation email that allows you to change the status of your specific appointment. It's extremely easy. If there are any issues, just call, text, e-mail, or send a message from the website with your first and last name and request and we'll do the rest.
• We do not give prescriptions for benzodiazepine medications such as alprazolam (Xanax), lorazepam (Ativan), diazepam (Valium), clonazepam (Klonopin).
• We may not give stimulant medications (typically used for Attention Deficit Disorder) on the first visit even if it was prescribed by a previous provider.
• If you are a NEW PATIENT and schedule your first appointment as a FOLLOW UP your appointment will be automatically canceled.
