2. Current Drivers License or State ID (Out of state accepted with proof of IL residency)
3. Proof of residency (IF address on ID is NOT current)
4.SSI or SSDI annual letter stating name, date and type of Social security you are on
5. Passport 'styled' Photo (If you do not have this, we can take your picture)
6. Credit or Debit card for online payment
