-Schedule this appointment if you have not been in for more than 9 months or were directed by the provider to schedule
-Includes an adjustment and a brief exam/consultation
-1 complimentary re-exam included in the 12 visit plan, 2 complimentary re-exams included in 25 +visit plan
- If you currently have plan and have used all of your complimentary re-exam visits , you may choose 2 visits to be subtracted from your plan.
