DUE TO COVID-19 RESTRICTIONS, TESTING IS AVAILABLE AT THE WATERLOO LOCATION ONLY.
PLEASE MONITOR THE WEBSITE REGULARLY FOR UPDATES TO THE TESTING SCHEDULE. SHOULD YOU REQUIRE TESTING BUT DO NOT SEE AVAILABLE DATES, PLEASE EMAIL email@example.com . YOUR EMAIL SHOULD INCLUDE THE NAME OF THE SERVICE THAT YOU ARE TESTING FOR, AND, THE DATE THAT YOU REQUIRE THE TEST TO BE COMPLETED BY.
If you require testing, please do NOT delay in booking your test.
Given the current public health crisis, we do not know if, or when, the Ontario Public Health Office might issue an order to postpone or stop testing. For this reason, unless you are under quarantine or are symptomatic, rescheduling of current appointments to later dates is STRONGLY discouraged.
When candidates present for testing, they will be required to perform hand hygiene and COVID-19 screening before proceeding to intake and testing.
This is an extremely difficult time for everyone. The OPPAT team very much appreciates your patience and understanding over the next few weeks.
CHANGING / RESCHEDULING AN APPOINTMENT: If you have registered for a test date and time and would like to change / reschedule the date or time, please use the "cancel / change your appointment" button on your confirmation receipt that was emailed to you at the time of registration. DO NOT USE THE CANCELLATION option. There is no fee to change appointments that have already been booked. Rescheduling appointment times is subject to availability. OPPAT reserves the right to cancel and/or reschedule appointments as needed.
CANCELLING AN APPOINTMENT: If you need to cancel your appointment (i.e., you won't be coming for testing), please use the "cancel / change your appointment" button on your confirmation receipt that was emailed to you at the time of registration. If you cancel your appointment 72 hours prior to the start of the scheduled testing date and time, your registration fee will be fully refunded and no cancellation fee will be applied. If you cancel your appointment with less than 72 hours from the date and time of your appointment, or you do not show up for your testing appointment, your registration fee will not be refunded in part or in whole.
HOW TO PREPARE FOR YOUR TEST:
BEFORE THE DAY OF TESTING:
Candidates MUST download, print, complete, and sign the PAR-Q form that can be accessed on the OPPAT website; all four (4) pages MUST be brought to their appointment:
Candidates with a resting blood pressure that is above 150/95 mmHg, and/or a resting heart rate above 100bpm must have written medical authorization from their physician or other regulated healthcare practitioner that clearly indicates that the candidate can participate in physical demands testing.
Candidates who have a pre-existing medical condition or who experience 'white coat syndrome' are strongly encouraged to bring medical authorization from their healthcare provider.
Female candidates who may be pregnant or who have a confirmed pregnancy are also required to complete the eParMed-X+ (see link below) and they must also have written medical authorization from their physician or other regulated healthcare practitioner that clearly indicates that the candidate can participate in physical demands testing http://eparmed-x.appspot.com/?locale=en#pub/parmedx
ALL medical documentation signed by a regulated healthcare provider must be dated within 30 days of the candidates’ appointment.
Candidates who do not follow the above instructions will have their test cancelled; NO refund will be issued. NO EXCEPTIONS.
